The above-named applicant is my dependent, and they live in my home. If they are selected to be a CEC Youth Tour recipient, I grant unrestricted permission for my dependent's name and image to be used in print, video and digital media. I agree CEC may use these images for a variety of purposes, and they may be used without further notification.
